About the Position:
Hyde Park Assisted Living is seeking an experienced RN or LPN to join our leadership team as Director of Patient Services.
This is a senior clinical leadership position responsible for overseeing the clinical operations of our assisted living community. The Director will work closely with administration, residents, families, physicians, and staff to ensure high-quality resident care and regulatory compliance.

Responsibilities
- Oversee the day-to-day clinical care of residents.
- Conduct and oversee resident assessments, care plans, and clinical documentation.
- Monitor residents for changes in condition and coordinate appropriate follow-up.
- Communicate with physicians, NPs, pharmacies, hospitals, families, and outside healthcare providers.
- Oversee medication management, MARs, medication orders, and routine medication audits.
- Oversee the clinical portion of new resident admissions.
- Supervise nursing and direct-care staff.
- Assist with hiring, training, scheduling, evaluations, and staff development.
- Monitor falls, incidents, medication errors, and other clinical events.
- Conduct quality assurance and clinical documentation audits.
- Ensure compliance with applicable regulations, policies, and clinical standards.
- Identify opportunities to improve resident care and clinical operations.
- Serve as a clinical leader and resource for the nursing and care team.
Qualifications
Required:
- Current New York State RN or LPN license in good standing.
- Strong clinical assessment and documentation skills.
- Strong communication and organizational skills.
- Ability to supervise, lead, and hold staff accountable.
Preferred:
- 3 years of nursing experience.
- Previous nursing leadership or supervisory experience.
- Experience in assisted living, adult home, long-term care, home care, or a similar setting.
- Experience with medication management and regulatory compliance.

Compensation
