What You’ll Do:
§ Conduct comprehensive MDS assessments and ensure compliance with federal and state regulations.
§ Collaborate with interdisciplinary teams to create and update individualized care plans for residents based on assessments and resident needs.
§ Train and support nursing and rehab staff in MDS processes and documentation.
§ Review and analyze data to maintain quality assurance and improvement in resident care.
§ Participate in care conferences with residents and families to discuss care plans and goals.

Qualifications
· Current and valid Registered Nurse (RN) license in the State of Washington.
· Experience in long-term care and knowledge of the MDS process is preferred.
· Strong assessment, organizational, and communication skills.
· Ability to work effectively in a team-oriented environment and foster positive relationships with residents, families, and staff.
· Certification in MDS (e.g., MDS Coordinator certification) is a plus.
· Must be able to pass a background check and meet all state requirements
Salary : $48 - $52
