Site Physician – Day Treatment Center (DTC)
Location: Brooklyn, NY (Rockaway Parkway & Bushwick)
Schedule: Monday–Friday | 8:00 AM – 5:00 PM (1-hour lunch)
Type: Full-Time | Contract with potential for temp-to-perm
We are seeking a Site Physician to provide high-quality, patient-centered care within a Day Treatment Center setting serving adult and geriatric populations. This is a full-time opportunity offering a consistent weekday schedule and the potential to transition into a permanent role.
Under the direction of the Regional Medical Director, the Site Physician will oversee the clinical, administrative, and organizational aspects of care delivery, ensuring excellence in patient outcomes and compliance with regulatory standards.
What You’ll Do
- Provide comprehensive primary care to adult and geriatric patients
- Conduct routine exams, assessments, and follow-up care
- Manage chronic conditions and coordinate specialty care as needed
- Collaborate with interdisciplinary teams including nursing, social work, and rehab services
- Maintain accurate, timely documentation in compliance with Medicare/Medicaid guidelines
- Participate in care planning and case conferences
Must-Have Qualifications
- Board Certified Physician (MD or DO)
- Active, unrestricted New York medical license
- DEA license
- Medicare & Medicaid provider IDs
- Experience in Geriatrics, Primary Care, or Internal Medicine
- Strong clinical judgment and documentation skills
Nice to Have
- Bilingual in Spanish or Haitian Creole
- Experience in Day Treatment Center (DTC), PACE, or managed care settings
- Comfort working with medically complex, underserved populations
