As a Home Health LPN, you will provide skilled nursing care under the direction of an RN and physician-established plan of care, including:

• Complete skilled nursing visits in patients' homes

• Monitor and document patients' conditions

• Provide medication management and education

• Perform wound care and other ordered treatments

• Monitor vital signs and report significant changes

• Reinforce disease-management education

• Educate patients, families, and caregivers

• Communicate changes in condition to the supervising RN and care team

• Coordinate with other members of the interdisciplinary team

• Follow established plans of care and physician orders

• Complete accurate and timely documentation using WellSky

• Active Virginia Licensed Practical Nurse (LPN) License Required

• Graduate of an accredited Practical Nursing program

• Valid Driver's License and reliable transportation

• Strong clinical and communication skills

• Dependable, organized, and comfortable working independently in patients' homes

• Ability to follow established plans of care and communicate effectively with the supervising RN

• Home Health Experience Preferred, but Not Required

• WellSky Experience is a Plus
