Monitor patient condition and report changes
Educate patients and caregivers

Follow safety and infection control standards
Qualifications Active LPN/LVN License
2 years nursing experience (home health preferred)
Strong assessment and communication skills
EHR proficiency
Valid driver’s license and reliable transportation
