What You'll Do
- Provide skilled nursing care under the direction of a Registered Nurse and physician
- Administer medications and perform treatments as ordered
- Monitor patient conditions and report changes to the supervising RN
- Assist with disease management and patient education
- Complete wound care and other skilled nursing procedures within your scope of practice
- Collaborate with physicians and the interdisciplinary care team
- Complete timely and accurate clinical documentation
Qualifications
- Active Virginia Licensed Practical Nurse (LPN) License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical, Communication, and Documentation Skills
- Home Health Experience Preferred but Not Required
