Human Touch Home Health is seeking a Full-Time Physical Therapist (PT) to join our Blacksburg team serving patients throughout Blacksburg, Christiansburg, Radford, and surrounding New River Valley communities.

What You'll Do
- Complete physical therapy evaluations in patients' homes
- Develop individualized plans of care
- Provide therapeutic exercise, gait, balance, transfer, and mobility training
- Assess home safety and fall risks
- Educate patients and caregivers
- Monitor patient progress and adjust treatment plans
- Complete progress assessments and discharge planning
- Coordinate with physicians, nurses, PTAs, and other members of the care team
- Complete accurate and timely documentation using WellSky
Qualifications
- Active Virginia Physical Therapist License Required
- Graduate of an accredited Physical Therapy program
- Valid Driver's License and reliable transportation
- Strong clinical and communication skills
- Ability to work independently in patients' homes
- Home Health and OASIS experience preferred, but not required
- WellSky experience is a plus
