Description of Position:
The Support Coordinator is responsible for the monitoring of activities and supports pertinent to the supports provided by New Horizons to the participants assigned to their caseload. A key responsibility will be to work closely with all stakeholders in achieving ongoing excellence in supports provided by New Horizons in compliance with all policies and procedures, state and federal regulations, and requirements of CQL accreditation.
Position Details:
Full Time: 40 Hours per Week; Monday - Friday
Shift: 8:00am to 4:30pm
Travel Required: Once in awhile for training.
Pay Range: $17 - $18

Role and Responsibilities:
· Responsible for maintaining and upkeep of people’s files and records.
· Serves as the primary contact person to the person on assigned caseloads when assistance is required.
· Participates in all planning meetings, assisting with the Person Centered Planning, and implementing team recommendations.
· Delegates the completion of assessments as assigned by the Case Manager.
· Provides input related to the ICAP assessment to the Case Manager.
· Reviews completed support plan, ISP monitoring and ensures appropriate follow up with team members.
· Maintains a positive and professional working relationship with participants, families, guardians, staff, state agencies, case managers and other professionals.
· Communicates with the case manager to convene the team to review the person’s plan with problems or changes that call for review when indicated.
· Ensures that new people supported are properly orientated to the agency.
· Other duties as assigned consistent with non-exempt functions.
Qualifications:· High School Diploma or GED.
· Valid Driver's License and safe driving record.
· Successful completion of criminal background check.
· Must be able to pass a pre-employment drug test.
The Board of Directors of the Human Service Agency shall have the prerogative to hire staff whose qualifications may vary from the above. This position requires access to Electronic Personal Health Information (EPHI). Current HSA Policies and Procedures apply. It is the responsibility of the employee to read, understand, and comply with HSA policies and procedures that apply to protected information and security.
