Description of Position:
The mission of New Horizons is to support people to achieve self-reliance and to exercise control over their lives. The Direct Support Coordinator provides guidance and support to individuals to help them achieve their personal goals and desired quality of life while ensuring their health, safety, and well-being. This role also supervises staff and supports person-centered services that reflect each individual’s hopes, dreams, and goals.
Position Details:
- Employment Type: Full-Time, 40 hours per week
- Schedule: Monday–Friday with participation in an on-call rotation
- Shift Hours:
- Monday, Wednesday, and Friday: 8:00 AM – 5:00 PM
- Tuesday and Thursday: 11:00 AM – 7:00 PM
- Compensation: $50,000 annually
Job Knowledge and Comprehension:
- Understands and demonstrates a commitment to supporting people in achieving their hopes, dreams and wishes (personal outcomes).
- Serves as a member of the person support team, which designs the personal support plan for each person and attends the meetings.
- Ensure people’s health, safety and welfare is not compromised through unreasonable or unnecessary risk.
- Monitors needed supports for people supported.
- Helps people manage funds in a responsible manner.
- Acts as an advocate for people making sure that people’s rights are not violated and ensuring needed supports are secured.
- Knowledgeable of assessment and other processes used to discover people’s hopes, dreams, wishes, and preferences. Uses the information that is discovered from the assessments and other processes to help develop an Individual Support Plan.
- Monitors crisis situations, discusses the incident with authorized staff and people supported. Documents information from pertinent conversations with others. Request team meetings to adjust supports as needed to help the person remain safe and healthy.
- Knowledgeable of and complies with regulations for reporting to the Division and other agencies.
- Serves as a liaison for people, their families, and guardians.
- Provides leadership and supervision to all Direct Support Professionals to ensure compliance with accreditation standards and agency policies and procedures.
- Provides services, supervision, and accountability of services and activities for people to ensure each is protected from physical, emotional, sexual, or psychological abuse or mistreatment.
- Provides services to people and Direct Support Professionals by collaborating with them to ensure that all needed supports are met. This may include, but is not limited to, direct cares of people.
- Ensure that there is a proper amount of staff assigned to shifts as recommended by the Division of Developmental Disabilities and the management staff at the Human Service Agency.
- Ensures that all staff working have training in all areas required by ARSD rules.
- Other duties as assigned consistent with exempt functions.
Physical Requirements:
The employee frequently is required to sit and work extended periods of time at a computer monitor. Occasionally, this position may require lifting that exceeds 50 pounds during medical emergency situations and/or behavior support. Job requires bending, stooping, climbing stairs, pushing/pulling, and driving a vehicle.
Qualifications:High School Diploma or GED required.
Two years' experience working with people with Disabilities is preferred.
Supervisory experience is preferred.
The Board of Directors of the Human Service Agency shall have the prerogative to hire staff whose qualifications may vary from the above. This position requires access to Electronic Personal Health Information (EPHI). Current HSA Policies and Procedures apply.
