Job Knowledge and Comprehension:

- Monitors individual support needs and assists with personal funds in accordance with established procedures.
- Advocates for people served and helps ensure appropriate services and supports are provided.
- Uses assessment information to support person-centered planning and the implementation of Personal Support Plans.
- Complies with applicable regulations and reporting requirements for the Division of Developmental Disabilities and other agencies.
- Serves as a liaison among people served, families, guardians, and other agencies to support communication and coordination of services.
- Develops and maintains client and staff schedules to support daily Community Services operations and appropriate staffing coverage.
- Prepares monthly sign-up sheets and enters scheduling requests and updates by the end of each business day.
- Updates staff schedules to address call-outs and other scheduling changes and communicates updates to appropriate staff.
- Assists with planning and organizing activities, special events, and community outings.
- Attends Individualized Service Plan (ISP) meetings and other team meetings as needed.
- Performs other duties as assigned that are consistent with the responsibilities and non-exempt classification of the position.
Physical Requirements:
The employee is frequently required to sit and work extended periods of time at a computer monitor. Occasionally, this position may require lifting that exceeds 50 pounds during medical emergency situations and/or behavior support. Job requires bending, stooping, climbing stairs, pushing/pulling, and driving a vehicle.
Qualifications:High School Diploma or GED Required.
Valid driver’s license and safe driving record.
Successful completion of criminal background check.
Must be able to pass a pre-employment drug test.
