Description of Position:

The Direct Support Professional is to provide the supports necessary for people to live and/or work independently while being involved, respected and included in their community. The Direct Support Professional is responsible to assist each person supported to achieve personal outcomes that reflect their dreams, hopes and desires.
Position Details:
Full Time: 40 Hours per Week; Alternating Weekends
Shift: 12:30pm to 9pm
Pay Range: $17- $18, plus shift differentials

Job Knowledge and Comprehension:
- Transportation of people supported to work, activities and appointments.
- Staff is knowledgeable of assessments and processes used to discover the needs, preferences, and capabilities of the persons supported and how that information is used in development and review of the support plan.
- Monitors crisis situations, discusses incident with authorized staff and people supported. Adjusts supports to individual preferences, helps maintain a safe environment, and complies with regulations for reporting.
- Teach persons supported to ensure their safety in their own homes (secure windows, doors, shut curtains, etc.).
- Supports and assists persons supported in all areas of their daily life, including but not limited to: personal appearance, laundry, menu planning, grocery shopping, home cleaning, communication skills, personal hygiene, finances, purchasing of personal items, clothing and home decorations.
- Monitors daily cleanliness and sanitation in the homes of people supported living in their own homes. Assist them in learning to keep their homes neat and clean.
- Teaches individuals living in their own homes what to do in case of fire, tornado or other emergencies.
- As needed will train the person supported in job seeking, job skills, hygiene and appearance and becoming an independent worker capable of performing job duties in an acceptable fashion.
- Maintains comprehensive records on each person’s job training process.
- Fades out of job training and supervision as appropriate and in a timely manner.
- Other duties as assigned consistent with non-exempt functions.
Physical Requirements:
Lifting requirements may exceed 50 lbs. during medical emergencies and/or behavior support. Other physical activities include transferring people we support to and from wheelchairs/beds/toilets, making beds, pushing wheelchairs, carrying groceries, vacuuming, housekeeping, sweeping, and laundry. Job requires bending, stooping, climbing stairs, and pushing/pulling. This position requires driving a vehicle frequently.
Qualifications:High School Diploma or GED.
Valid Drivers License and safe driving record.
Successful completion of criminal background check.
The Board of Directors of the Human Service Agency shall have the prerogative to hire staff whose qualifications may vary from the above. This position requires access to Electronic Personal Health Information (EPHI). Current HSA Policies and Procedures apply. Salary : $17 - $18
