JOB SUMMARYThe ACT MHP (Mental Health Professional) Case Manager is responsible for providing comprehensive, community-based services to adults living with serious mental illness, utilizing the Assertive Community Treatment (ACT) model. This position focuses on client engagement, recovery-oriented care, coordination of services, and ongoing support to help individuals achieve stability and independence within the community.JOB DUTIESComplete 6–7 billable client engagements dailyParticipate in a 24/7 on-call rotation for crisis interventionConduct biopsychosocial assessments and develop IM CANS documentationCoordinate services across housing, medical, vocational, and mental health domainsProvide outreach, supportive counseling, and problem-solving strategiesCollaborate with internal teams and external agencies for discharge planningEnsure all documentation meets Medicaid, agency, and state standardsMaintain accurate and timely DIS billing for services renderedPromote mental health awareness through education and outreachQUALIFICATIONS Bachelor’s degree in Social Work, Psychology, Counseling, or a related human services field (Master’s preferred)Two years of experience in community or behavioral health preferredValid driver’s license, reliable transportation, and ability to travel within service areaFamiliarity with DSM-5, ASAM, and Medicaid-funded programsEHR proficiency and strong communication/documentation skillsCulturally competent; bilingual (especially Spanish) preferredSMART GOALSClient Stability: 90% of assigned clients maintain housing and avoid hospitalizationTreatment Compliance: 100% of treatment plans completed on time with measurable goalsResource Utilization: Build relationships with 3 new community resources quarterlyWORKING CONDITIONS AND PHYSICAL REQUIREMENTSTravel throughout the community requiredMay require lifting 10–25lbs, walking/standing for extended periodsMinimal physical riskEMPLOYMENT CONDITIONSThis job description is to ensure that essential functions and basic duties have been included.
