Summary:Care Coordinators maintain responsibility coordinating and providing services for adults/children who have been diagnosed with a mental illness. This position works with a multi-disciplinary team of internal and external providers.
Duties:- Accept assigned referrals and engage clients to initiate services.
- Help navigate internal and external health services.
- Assist in the client's integrated treatment coordination, which may include treatment planning, completing release of information(s), progress notes, and referrals to internal and external providers.
- Coordinate client services in areas of mental health, chemical dependency, housing, financial benefits, insurance benefits, employment, transportation, education, and other basic needs.
- Two or more years of experience providing mental health services to clients.
- Experience navigating health, employment, housing, and/or insurance systems.
Supervisory experience.
