What are the responsibilities and job description for the Nurse Practitioner (Post-Acute Care) position at HSM Worx?
Passaic County, NJ Full-time / Part-time On-Site Day Shift
About the Role
We’re recruiting Nurse Practitioners for a new physician-led medical practice that cares for post-acute patients in skilled nursing facilities in the Passaic County area.
You'll round on subacute rehab and long-term care patients, assess new admissions and acute changes, and work with a collaborating physician and facility teams to keep patients out of the hospital when they don't need to be there. The practice uses AI-assisted documentation, so less of your day goes to charting.
This is a chance to join a practice at the start and help shape how it runs. It's a fit for an experienced NP who wants autonomy with physician support, enjoys medically complex patients, and wants a sustainable schedule.
What You'll Do
- Assess new admissions and acute issues for subacute and long-term care patients.
- Round on your patient panel across facilities in your region.
- Catch changes in condition early and intervene to prevent avoidable hospital transfers.
- Manage chronic conditions and adjust treatment plans as patients' needs change.
- Work closely with the collaborating physician on clinical decisions and complex cases.
- Communicate and coordinate with specialists, primary care providers, and facility nursing and administrative teams.
- Document visits using the practice's AI-assisted documentation tools.
- Help shape the practice's workflows and standards of care as it grows.
What You'll Bring
- Active New Jersey Nurse Practitioner (APN) license.
- National board certification as an AGPCNP, AGACNP, or FNP.
- 2 years of experience as a Nurse Practitioner.
- Background in internal medicine and/or geriatrics.
- Hospital experience. [Preferred]
- Post-acute or skilled nursing facility experience. [Preferred]
- Strong assessment and critical thinking skills, and comfort working in a fast-paced setting.
- Clear written and verbal communication, and comfort with EHRs and digital tools.
- Ability to work as part of an interdisciplinary team.
Compensation & Benefits
- $135,000 to $145,000 per year.
- Full-time and part-time schedules available, with flexible hours and days for part-time.
- Health, dental, and vision insurance.
- 401(k) retirement plan.
- Paid time off.
Who You Are
You're an experienced NP who likes the pace and variety of post-acute care and is comfortable making decisions with a physician a call away. You notice small changes before they turn into hospital transfers, you work well with facility staff, and you'd like a real say in how a new practice runs.
