About the Role
Our client, a skilled nursing and sub-acute rehabilitation facility in Flemington, NJ, is seeking a part-time Licensed Social Worker to join its team. This is a direct-hire position with the facility.
You’ll be part of the facility’s social services team, supporting residents and families from admission through discharge. The work covers psychosocial assessments, MDS documentation, care planning, benefits guidance, and discharge coordination, alongside nursing, rehab, and the rest of the interdisciplinary team.
This role fits a licensed social worker who knows post-acute care and wants flexible part-time hours in a setting where the work makes a visible difference for residents and their families.
What You’ll Do
- Complete psychosocial assessments for new admissions and ongoing care
- Help new residents settle in and adjust to the facility
- Complete MDS 3.0 documentation within state and federal guidelines
- Participate in interdisciplinary care plan meetings
- Guide residents and families through Medicaid applications, insurance, and financial resources
- Advocate for resident rights and provide crisis intervention when needed
- Coordinate safe discharges with families, home health teams, and community partners
What You’ll Bring
- Active New Jersey CSW or LSW in good standing
- Experience working with older adults and individuals with complex physical or cognitive needs
- Strong clinical documentation skills and attention to accuracy
- Strong organization and time management
- 1 year in a skilled nursing or sub-acute rehab setting [Preferred]
