What are the responsibilities and job description for the Direct Support Professional position at HS&M Mental Health & Education?
Do you reside in or near Apex, Durham, Wake Forest ideally located near Raleigh. Are you passionate about making a direct impact on the lives of individuals with intellectual and developmental disabilities, serious mental illness or substance abuse disabilities? HS&M is seeking compassionate and motivated individuals to provide home and community-based support in the community.
We have openings for team members to provide the following services:
- Community Living & Support (CLS): Assist individuals with developing and enhancing self-help, socialization, and adaptive skills to live successfully in their own homes and participate in community life.
- Supported Employment (SE): Help individuals obtain and maintain competitive employment in integrated community settings. Provide job coaching and on-the-job support.
- Respite: Provide temporary, non-medical care to relieve primary caregivers, ensuring the individual's safety and well-being during engagement in recreational and social activities.
Key Responsibilities:
- Provide one-on-one support tailored to individual needs and goals.
- Encourage community integration and participation.
- Assist with daily living activities and personal care (for CLS/Respite).
- Document services provided accurately and in a timely manner.

Requirements:
- High School Diploma or equivalent.
- Valid Driver’s License & reliable transportation.
- Compassion, patience, and a positive attitude.
- Previous experience is a plus but not required.
