About the Opportunity
Trajectory Recruiting is partnering with a respected healthcare organization in Milwaukee County to identify a compassionate and experienced Primary Care Physician. This physician will provide comprehensive medical care to an established panel of adult and geriatric patients while collaborating with a multidisciplinary healthcare team to deliver exceptional, patient-centered care.

Key Responsibilities
- Provide comprehensive primary care, including preventative, acute, and chronic disease management.
- Perform physical examinations, health assessments, and develop individualized treatment plans.
- Diagnose and treat a wide range of medical conditions using evidence-based practices.
- Order, interpret, and review laboratory tests, imaging, and other diagnostic studies.
- Prescribe and manage medications while monitoring treatment effectiveness.
- Coordinate specialty referrals and follow-up care as needed.
- Collaborate with nurses, advanced practice providers, therapists, social workers, and other healthcare professionals to ensure coordinated patient care.
- Educate patients and caregivers on disease prevention, medication management, and healthy lifestyle practices.
- Maintain accurate and timely medical documentation within the electronic medical record.
- Participate in interdisciplinary care planning and quality improvement initiatives.
- Provide clinical oversight and supervision to advanced practice providers when appropriate.
Qualifications
Required
- MD or DO from an accredited medical school.
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Active Wisconsin medical license (or ability to obtain prior to start).
- Current DEA registration and National Provider Identifier (NPI).
- Current CPR certification.
- Minimum of one year of recent experience providing primary care to elderly, geriatric, or medically complex adult populations.
- Strong clinical judgment with experience managing multiple chronic conditions common in older adults.
- Excellent communication, documentation, and interpersonal skills.
Preferred
- Board Certification in Geriatric Medicine.
- Experience working within a multidisciplinary or integrated care environment.
