Compensation Range: $24/hr - $26/hr commensurate with experience

Overview: 

This role is within a Ryan White-Part A program working with individuals who are HIV Positive. This role requires focus on HIV care coordination, viral load suppression, and harm reduction strategies to support participants’ viral load suppression and wellness. The Case Manager will conduct eligibility screenings, enrollment interviews, care plan development, care coordination, referral services, coordination of substance use counseling, health education, and social services navigation to participants with substance use and other chronic health conditions. Other interventions are Healthy Conversations, AOD Counseling and Seeking Safety education as eligibility allows.

The Case Manager will act as an intermediary between care providers and members – serving in numerous roles such as educator, evaluator, service coordinator, community resource researcher, advocate, and more. This role will work with Ryan White and Harm Reduction prevention services for HIV populations.

 

Responsibilities:

Primary:

  • Completes initial assessment and reassessments on all assigned clients in a timely manner in-order to develop individualized treatment goals for each client.
  • Establishes treatment goals and objectives with clients as part of the interdisciplinary team. Constantly assessing the client’s ability to succeed and navigate through challenges difficulties.
  • This role will work closely with the participant’s primary care setting and health care providers as needed to optimize positive health outcomes.
  • Assists clients to navigate the health insurance system, mainly the AIDS Drug Assistance Program (ADAP), Medicaid, and/or other insurance networks.
  • The case manager will engage clients to complete quarterly assessments, quarterly care planning, counseling, outreach, care coordination, case conferences, advocacy, referrals (on-site and off-site), and follow up discharge planning goals from detox or other inpatients facilities.
  • In a manner consistent with Harm Reduction principles, provide substance use and sexual risk assessments and discuss individualized risk reduction strategies, including safer injection practices, safer sex practices, overdose prevention, HCV, buprenorphine and other topics relevant to people who use drugs. • Conduct overdose prevention education and Narcan dispensation to program participants as needed.
  • Facilitates patient-centered individual and group services, including crisis management, risk reduction, treatment readiness, overdose prevention, HIV health and medication management, and other support and educational groups.
  • Attends weekly/bi-weekly supervision meetings as agreed upon by the Program Coordinator.
  • Ensure timely submission of progress notes, reassessments, service plans, clinical documentation of services, and other program deadlines to avoid interruptions in services with program participants. Documentation must align with the program’s scope and definition of services which is entered into our Electronic Health Record system.
  • Work collaboratively with team members to provide home outreach and engagement for participants who become disconnected from services.
  • Conduct home visits, field work and other provider settings as needed to fulfill goals in the care plan.

Secondary:

  • Participate in regular case conferences with medical providers and other stakeholders as appropriate to the care and treatment of the client.
  • Provides weekly and monthly clients’ progress summaries as needed.
  • Aid through advocacy and psychoeducation to clients and family for entitlements and other community services.
  • Understands and is sensitive to chronic medical conditions, including HIV, mental health and substance use, diabetes, asthma, hypertension, heart disease and hepatitis diseases.

  • Performs other duties as requested
  • Participate in quality assurance and quality improvement planning project activities.
  • Participates in monthly staff development meetings and interventions. 
Job Requirements:
  • Preferably a bachelor's degree in the human services field with at least two years of case management or social services experience.
  • Minimum of a High School diploma with no less than 5 years of proficiency in case management and/or care coordination services experience.
  • Minimum of 2 years of experience of case management with Ryan White eligible populations and navigating through socio economic challenges encountered by marginalized and high-risk populations.
  • We are specifically looking for candidates with an active interest in harm reduction, syringe exchange, and/ or social justice as it pertains to individuals who use drugs.
  • Ability to make data entries into programs database such as AIRS, e-Share, EiCare
  • Use of computer software --Microsoft Office Suite including Word and Outlook calendaring, and Excel.
  • Good group facilitation skills and overdose prevention training experience
  • Skilled with health insurance navigation and documentation.
  • Good verbal, written, communication and interpersonal skills
  • Culturally aware and sensitive to PWUD (People Who Use Drugs), PLWHA (People Living with HIV/AIDS), LGBTQAI, YIDU (Young Intravenous Drug Users), SMI (Severe Mental Illness), and MSM (Men who have Sex with Men)
  • Experience working with the adult population who has co-occurring substance use and non-chronic mental health disorders.
  • Knowledge of community health and social service agencies, public benefits and additional community resources.
  • Preferable training in the following areas:
    • Risk Reduction Counseling
    • HIV 101 and Viral Hepatitis 101
    • Motivational Interviewing
    • LGBT Sensitivity
    • Syringe Exchange and Harm Reduction Training
    • HIV confidentiality and risk-reduction counseling
    • Overdose Prevention Strategies
  • Bilingual in Spanish/English preferred
  • A good understanding of what Harm Reduction is and how it is used in case management.
  • Creative problem solving and critical thinking in navigating complex social issues.
  • Excellent time management, organization and consistency in standards of documentation.
  • Frequently remaining in a stationary position, often standing, or sitting for prolonged periods
  • Occasionally moving around to other worksites
  • Occasionally lifting and moving objects up to 25 pounds in ordinary office related tasks.
