What are the responsibilities and job description for the CSP Case Manager position at HOUSING FAMILIES INC?
Role Purpose: Reporting to the Community Supports Program Manager the CSP-HI Case Manager will build relationships with populations experiencing homelessness, and assist with basic needs assessments, housing search, access to public benefits and other resources as appropriate.
Responsibilities:
- Provide thorough screening for CSP-HI eligibility for all clients on a regular basis. Consistently engage with new clients as current clients end services.
- Provide direct services and build relationships with unsheltered individuals on the street, in encampments, in inpatient treatment facilities or other designated locations. Some meetings will also take place in the office.
- Services will include performing basic needs assessments, delivering food and hygiene items, and offering various resources and referrals.
- Conduct thorough housing assessments and assist all clients in completing housing applications, mitigating barriers to housing, and gathering required documentation.
- Assess all populations for crises needing immediate intervention and connect them to internal and/or external support.
- Collaborate with existing community service providers to maximize resources and benefits available to all populations.
- Advocate for clients in various capacities.
- Utilize a Trauma Informed Care model which values autonomy, harm-reduction, etc. Ensure all individuals served are treated with dignity and respect.
- Ensure all services are provided in compliance with various funding sources.
- Complete all documentation and reports in a thorough and timely manner.
- Communicate well with supervisor, colleagues, and collaterals. Able to receive and implement feedback.
- Other duties as assigned.
Qualifications: Must have experience working directly with clients in the human services field. Must have working knowledge of statewide housing opportunities, public benefits and local resources. Working knowledge of the impact of trauma.
Must have strong organizational and time management skills. Must be flexible, able to work in various envronments, and possess excellent communication skills. Proficient with Microsoft Office Suite and ability to quickly learn new software and tracking systems. Working knowledge of domestic violence, substance use and mental health conditions. Bilingual Spanish is a plus.
Eligibility Requirements:
- Must be willing to work in Metro Boston/North Shore and have reliable transportation.
- Must be willing to work and 8-hour shift between 8am-5pm Monday to Friday and some evenings to outreach those in overnight centers is required. Or an 8-hour shift 4pm-12am for those specifically in overnight centers.
- Must be willing to work outside in various weather conditions.
- Must have unrestricted work authorization to work in the United States.
- Must be 18 years or older.
