  • Develop individualized service plans that address client goals related to recovery, prenatal/postpartum health, parenting, and family stability.
  • Provide ongoing case management sessions to monitor client progress and adjust goals as needed.
  • Assist clients in obtaining prenatal care, primary care, behavioral health services, WIC, SNAP, childcare services, legal aid, and other needed community resources.
  • Conduct targeted outreach to pregnant and parenting women with SUD and related risk factors in the community, including outreach at clinics, hospitals, social service agencies, probation offices, and other community settings.
  • Develop and distribute outreach materials (flyers, brochures, presentations) to reduce stigma and increase awareness of services.
  • Build and maintain relationships with referral sources (e.g., OB/GYN offices, FQHCs, hospitals, WIC offices, DCFS, courts, community agencies) to promote program services and streamline referrals.
  • Participate in community events, health fairs, and coalition meetings to share information about the program and increase visibility and access for PWWDC.

Qualifications

  • Bachelor’s degree in social work, human services, psychology, or related field required.
  • Experience working with pregnant women, families, or individuals with substance use or mental health disorders preferred.
  • Experience in outreach, community-based work, or engagement with high-risk or underserved populations preferred.
  • Knowledge of community resources for maternal health, childcare, housing, and family support.
  • Ability to pass required background checks and drug screening.
