Duties

- Coordinate with interdisciplinary teams to facilitate comprehensive discharge planning, case management, and family education on disease progression and comfort measures.
- Provide emotional support and education to patients and families about hospice services, advanced directives, and symptom control options.
- Document all patient interactions accurately using electronic health record (EHR)
Experience
- Demonstrated proficiency with EMR/EHR system
- Ability to perform comprehensive physical examinations and diagnostic evaluations while maintaining high standards of patient safety.
- Excellent communication skills for educating families on end-of-life care options and collaborating effectively within multidisciplinary teams.
