Responsibilities
- Conduct comprehensive patient assessments to develop and implement individualized care plans.
- Maintain accurate documentation and communicate effectively with patients and caregivers
Qualifications
- Active RN License
- Strong nursing and case management skills

Pay: $40.00 - $45.00 per hour
