The Supportive (Palliative) Care Nurse Practitioner is a primary member of the Supportive practice and will play a vital role in building a support network for people with advanced, complex illness in and around Calvert County. The nurse practitioner will treat patients with cancer, chronic illness or dementing and debilitating diseases and must be willing to see patients in a variety of settings including traditional homes, nursing rehab facilities, assisted living facilities or any place of residence, and our outpatient supportive care clinics.

Job Duties
- Provide initial consultation and follow up visits as appropriate in hospital, nursing rehab, inpatient units, outpatient Supportive Care Center or place of residence
- Assess symptom burden, and craft treatment plans to reduce unnecessary suffering.
- Participate in advanced care planning discussions including recommendations for resuscitation status with supportive documentation and billing
- Lead family meetings
- Provide coaching in behavioral change or mind-body techniques to reduce illness exacerbations.
- Respond to phone questions from patients, families or members of the interdisciplinary team, including hospice nurses, for treatment options.
- Document and bill appropriately for services.
- Conduct in-service offerings for appropriate staff and community partners to raise awareness of Supportive Care issues and criteria for appropriate patients.
- Provide on call services as assigned
- Develop and maintain relationships with the hospital and/or facility staff.
- Meet with providers in private practice to develop and maintain referring relationships.
- Authorize prescriptions through the electronic system, complete electronic death certifications, and advise nursing and referral sources as required.
- Other duties as assigned by the Chief Medical Officer or Director of Chesapeake Supportive Care
Salary : $100,000 - $140,000
