We are currently seeking a full-time and a part-time Social Worker to join our interdisciplinary team and provide meaningful care and emotional support to patients and their loved ones during end-of-life care.
The full-time role is a field position, with a primary assignment throughout the Mid-Western Orange County. The work schedule for this position will be Monday-Friday, 8:30 am - 4:30 pm, with a Sunday rotation. The salary for this full-time role is $62,353.00 - $65,000 per year.
The part-time role is also a field position, with a primary assignment that spans the Greenwood Lake, Warwick, Pine Island and Florida region. The work schedule for the part-time position will be Monday, Wednesday, and Friday, 9:00 am - 3:00 pm, totaling 18 hours per week. (The part-time position is non-exempt and is not eligible to participate in our benefit plans.) The rate of pay for this part-time position is $30 - $35.23 per hour.
The Social Worker must possess a deep understanding of the hospice philosophy and the ability to effectively educate patients and families about available services. Some key responsibilities include conducting in-home visits, completing comprehensive psychosocial assessments, delivering clinical counseling and case management support, collaborating with the other members of the interdisciplinary team to develop and implement individualized care plans, and offering bereavement counseling and emotional support to our patients and their families.

Requirements
Education and Experience:
- A graduate of a master’s program in social work accredited by the Council on Social Work Education.
- Valid NYS Licensed Master Social Work/Licensed Clinical Social Work.
- Excellent verbal and written communication, and organization skills.
- Knowledge of and commitment to hospice philosophy.
- Ability to work as a team member.
- Ability to utilize technological devices required for documentation and communication with the Interdisciplinary team.
- Ability to work collaboratively within an Interdisciplinary Team.
