POSITION SUMMARY
The Hospice Admissions Coordinator is responsible for leading and coordinating all aspects of the admissions process,
ensuring patients and families experience a smooth, compassionate, and timely transition into hospice care. This role
manages patient referrals, oversees the admission process from initial inquiry to enrollment, and ensures accurate
and timely documentation in compliance with local, state, and federal regulations. A key part of this role includes
engaging with referral sources and guiding prospective patients and families to hospice admission when appropriate,
helping them understand the benefits and support available. The Admissions Coordinator provides leadership to the
Support Services staff, ensuring proper maintenance and compliance of all records. Serving as a liaison among
patients, families, referral sources, and the interdisciplinary team, this position fosters strong community and
professional relationships to support program growth and uphold the highest standards of care and service.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:
• Manage the intake process from initial inquiry through patient acceptance and start of care.
• Guide patients and families through the admission process, providing education, support, and reassurance.
• Schedule admission visits and coordinate referrals from hospitals, nursing facilities, or other care settings to
the patient’s home or hospice facility.
• Ensure eligibility criteria are met and that all required documentation is complete, compliant, and properly
recorded in the EMR.
• Meet with patients and families in various settings (home, hospital, office, nursing facility) to facilitate timely
admissions.
• Drive growth within service area by cultivating strong, trust-based relationships with physicians, hospitals,
nursing facilities, and other referral sources.
• Secure referrals and ensure all clinically eligible patients are rapidly admitted.
• Partner with clinical staff, liaison and operations to address service quality and responsiveness.
• Collect feedback from referral sources and share insights with leadership to improve services.
• Provide leadership and support to staff, ensuring consistent performance and adherence to policies.
• Train, mentor, and evaluate staff to promote professional development and accountability.
• Collaborate with leadership to streamline processes and enhance patient/family experience.
• Adhere to organizational policies and procedures, as well as state and federal hospice regulations.
• Maintain accurate, timely, and compliant documentation.
• Ensure all admissions activities reflect the hospice’s mission of compassion, dignity, and quality care.
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
• Strong interpersonal and communication skills (written and oral), including the ability to lead discussions in
diverse groups of varying size.
• Well versed and able to communicate with a high degree of competency regarding medical processes and
procedures involving hospice and palliative care.
• Hospice, palliative care, or healthcare admissions experience.
• Supervisory/leadership experience preferred.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work independently and manage multiple priorities.

of minimum coverage as follows: $100, ooo Per Person Bodily Injury, $300,000 Per Incident and $50,000
