The Registered Nurse (RN) will serve a dual role within the organization, providing skilled nursing care and coordination for both the Infectious Disease (ID) program and Chronic Care Management (CCM) services. The RN will work collaboratively with the Infectious Disease physician, managing patient follow-up, infusion coordination, and specialty case management, while also supporting chronic care patients with care planning, goal setting, patient education, and coordination across the care continuum. This position plays a vital role in improving health outcomes, enhancing patient satisfaction, and reducing hospitalizations for complex patient populations.

### **Duties/Responsibilities:**
**Infectious Disease (50%):**
Collaborate directly with the Infectious Disease physician to manage specialty patient care.
Coordinate lab results, imaging, treatment authorizations, and referrals.
Monitor patient progress and assist in managing patients on long-term antibiotic therapy or complex treatment regimens.
Schedule and coordinate infusions or treatments as needed.
Serve as a patient liaison for specialty pharmacy or infusion services.
Educate patients and families on disease process, infection prevention, medication compliance, and follow-up care.
**Chronic Care Management (50%):**
Provide ongoing support to patients with multiple chronic conditions such as diabetes, hypertension, COPD, CHF, etc.
Conduct regular telephonic or in-person check-ins with patients to support care plan adherence.
Develop individualized care plans in collaboration with providers.
Track health goals and coordinate with primary care and specialty providers.
Document all interactions and interventions in accordance with CMS CCM billing requirements.
Identify care gaps and coordinate appropriate services to improve health outcomes.
General Responsibilities
Maintain accurate and timely documentation in the Electronic Health Record (EHR).
Ensure HIPAA compliance and maintain patient confidentiality.
Participate in interdisciplinary care team meetings.
Actively support quality improvement and care coordination initiatives.
Perform other duties as assigned.
### **Education, Certifications, and Licenses:**
Current Registered Nurse (RN) license in the state of practice, in good standing.
Basic Life Support (BLS) certification required; ACLS preferred.
Bachelor's degree in nursing (BSN) preferred.
### **Experience:**
Minimum Of 2 Years Of Clinical RN Experience Required.
Experience in infectious disease, infusion therapy, or chronic disease management strongly preferred.
Experience with Electronic Health Records (EHR) systems, preferably Athena or Epic.
