Horizon PACE is excited to announce an amazing opportunity to be part of Horizon PACE’s program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare team at Horizon PACE will approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. Horizon has over 25 years’ experience providing care in Kentucky and is the largest provider of adult day health care services in the state. Horizon is opening 3 PACE locations in Kentucky, and we are excited to expand services to older Kentuckians. Under the supervision of the Horizon PACE Director, the Registered Dietitian screens and assesses participant nutritional status for Horizon PACE. Uses pertinent data to plan and implement appropriate nutrition interventions and communicates the information to the Interdisciplinary Team to ensure the nutritional needs of the participants are met. Performs nutritional assessment and provides nutritional education for Horizon PACE participants requiring interventions. Provides education to other health care professionals as appropriate. Assists in ensuring balanced meals are available, and assists as needed with service of meals.
QUALIFICATIONS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.
1. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
2. Must have a valid driver’s license, proof of insurance and have means of transportation.
3. Education: Bachelor’s or advanced degree from an accredited college with major studies in food and nutrition or dietetics required. Be legally authorized, currently licensed, registered, or certified if applicable to practice in the State of Kentucky.
4. Experience: Preferred to have aminimum of one year’s experience working with a frail or elderly population.
5. Competency: Position specific competencies for the Registered Dietitian will be met prior to assuming participant care.

ESSENTIAL FUNCTIONS:
1. Conducts an initial comprehensive nutritional assessment on all Horizon PACE participants upon enrollment. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
2. Conducts an in-person reassessment on at least a semiannual basis, or more often as needed, if determined to be actively involved in the development or implementation of the participant’s plan of care by the primary care provider, registered nurse or Masters-level social worker.
3. Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
4. Monitors monthly weights, medical diagnosis, drug nutrient interactions, changes in chewing, swallowing, mood changes, hydration status, intake, skin breakdown, reports changes to the Interdisciplinary Team and makes recommendations for changes to appropriate staff.
5. Documents nutrition notes and all other pertinent activities in the participant’s medical record according to Horizon PACE standards.
6. Provides nutritional counseling and education for Horizon PACE program participants and or caregivers using appropriate materials.
7. Provides nutritional educational programs to own department or other departments/Horizon PACE staff.
8. Assists, as needed, in monitoring meals, including intake, diet compliance and tray accuracy.
9. Develops and implements the nutritional components of participant plan of care including active participation and development of therapeutic dining programs.
10. Maintains supplementists and dietary census for Horizon PACE participants.
11. Ensures that the job responsibilities, authorities, and accountability of direct reports are defined and understood.
12. Responsible for the management of therapeutic diets, including modification to meet individual participant needs, and physician orders.
13. Acts within the scope of his or her authority to practice
14. Acts as liaison with contracted meal service to ensure meals meets the needs of the Horizon PACE participants.
15. Assists in development of policies and procedures for nutrition and dining services.
16. Protects privacy and maintains confidentiality of company procedures, results and information about employees, participants, and families.
17. Participates and support Quality Improvement Initiatives.
18. Performs other duties/projects and participates on other committees as requested.

e. Adheres to attendance policies, including clocking in and out for mealtime, following break schedule, and seeking supervisor’s approval prior to working overtime.
f. Completes requirements for in-service training, uniform and dress codes including personal hygiene, and other work duties as assigned.
5. Assist supervisor with various tasks to support department operations as directed; may also be assigned special projects.
6. Perform tasks which are supportive in nature to essential functions of the job, but which may be altered or re-designed depending on individual circumstances.
7. Assists participants and caregivers in filing grievances.

2. Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded with fluctuating indoor temperatures. Moderate pressure to meet scheduled appointments. Subject to people that may have the potential for verbal or physical aggression.
Physical Demands

Physical Requirements – Shall be in good mental and physical health, and capable of performing assigned tasks. Employee positions require strenuous activity including pushing, pulling, lifting of 40 pounds or more, standing for long periods, extensive walking, and movement at different position levels. Employment is contingent on the ability to perform these duties with no limits or restrictions according to regular work schedule.
