Job Title: Care Navigator – Hospice
Location: Hybrid (Remote with occasional in-office responsibilities as needed) Pay Range: $24.00 – $32.00 per hour (based on experience)
About the Role
We are seeking a compassionate, confident, and highly personable Care Navigator to serve as the first point of contact for patients and families exploring hospice care.
This role is primarily phone-based and remote, focused on delivering a high-quality, supportive experience through meaningful conversations. You will play a key role in educating families about hospice services, helping them understand their options, and guiding them through the initial stages of the hospice journey.
While much of the work can be performed remotely, this position also requires flexibility to support in-office needs as they arise, including occasional front-desk or receptionist-style responsibilities. This ensures a seamless experience for patients, families, and visitors across all points of contact.
Schedule
- Full-Time Position
- Monday – Friday
- 8:30 AM – 5:00 PM
Schedule may include a combination of remote work and in-office coverage based on team needs.
Key Responsibilities
PatientFamily Experience (Primary Focus)
- Serve as a welcoming, knowledgeable first point of contact for patients, families, and referral partners via phone and occasionally in person
- Provide clear, compassionate education on hospice philosophy, services, eligibility, and benefits
- Help families understand what hospice is-and what it is not with empathy and clarity
- Guide patients and families through the decision-making process, answering questions and addressing concerns
- Deliver a high level of customer service through professional, empathetic communication
Patient AccessIntake Coordination
- Receive and manage hospice referrals, ensuring timely response and follow-up
- Gather necessary information and documentation to support admissions
- Coordinate closely with clinical teams, social workers, and external providers to ensure a smooth transition into care
- Act as a bridge between initial inquiry and clinical admission
OfficeFront Desk Support (As Needed)
- Provide occasional in-office support, including greeting visitors and answering incoming calls
- Assist with front desk/reception coverage during peak times or team absences
- Ensure a professional, welcoming environment for patients, families, and partners
InsuranceBenefits Guidance
- Verify insurance coverage and hospice eligibility
- Explain benefits in a clear, easy-to-understand way for patients and families
- Support families in navigating financial and coverage-related questions
DocumentationCompliance
- Accurately document patient interactions and referral details in EMR systems
- Maintain compliance with Medicare, Medicaid, and state hospice regulations
Qualifications
- High school diploma required; Associate's or Bachelor's degree in healthcare or related field preferred
- 2 years of experience in healthcare, hospice, patient access, customer service, or receptionist/front desk roles
- Strong preference for candidates with phone-based and in-person customer interaction experience
- Comfortable working in both remote and in-office environments
- Exceptional communication, empathy, and interpersonal skills
- Ability to explain complex information in a clear, human-centered way
- Experience with EMR systems and basic computer proficiency required
- Ability to manage multiple priorities while maintaining a high level of service
