- Coordinate discharge planning, post-acute care services, and outpatient follow-ups to facilitate smooth transitions from hospital or inpatient settings to home or community-based care.
- Manage patient records meticulously, documenting assessments, interventions, and progress in medical records systems while maintaining compliance with HIPAA regulations.
- Engage in utilization management and diagnostic evaluation activities to ensure appropriate resource use and effective case management strategies.
- Provide education to patients and families about disease processes, medication management, and self-care techniques tailored to conditions like infectious diseases or trauma recovery.
Skills
- Skilled in patient assessment techniques including vital signs monitoring and diagnostic evaluation to inform case management decisions.
