What are the responsibilities and job description for the LPN Case Manager position at Compassion Home Care, Inc.?
This is not a remote position. This position is based in our Spiro office, Monday through Friday.
RESPONSIBILITIES AND DUTIES:
- Under direct supervision of the Clinical Director.
- Reviews Plan of Care developed by Registered Nurse.
- Continually re-evaluates patient needs and updates Plan of Care as needed.
- Notifies all disciplines involved in patient care of orders on Plan of Care.
- Keeps track of patient encounters in order to maintain correct frequencies.
- Develops and provides skilled nurse schedules on weekly basis.
- Attends scheduled case conferences and develops written documentation of case conference to develop Plan of Care.
- Communicates frequently with all disciplines involved in patient care in order to provide quality and continuity of patient care.
- Reviews and co-signs, as needed, all orders and provides copes of orders for all disciplines involved.
- Handles all incoming calls regarding patients under Case Manager’s care.
- Notifies physician of any abnormalities reported by field staff as needed.
- Notifies patient/family of changes in Plan of Care as they occur as applicable.
- Is available to all disciplines involved in patient care during business hours in case of problems or questions concerning patients.
- Performs other duties in absence of, or assistance to other staff, as requested.
- Participates in on-call rotation.
Job Requirements
- OK or Compact State LPN License in good standing with the Board of Nursing
- CPR Certificaiton (classes available after hire date)
