What You'll Do
As a HospiceHome Health Aide, you will provide compassionate, hands-on personal care to hospice patients in their homes and support families through respectful communication, comfort-focused care, and dependable assistance. Your key responsibilities include:
* Provide personal care services such as bathing, grooming, dressing, toileting, skin care, and other activities of daily living in accordance with the hospice plan of care
* Support patient comfort, dignity, and safety during each visit
* Observe and report changes in the patient's condition, comfort level, skin integrity, or family needs to the hospice nurse or case manager
* Provide simple meal assistance, light homemaking related to patient care, and support with maintaining a clean and comfortable care environment
* Offer compassionate presence and respectful support to patients and families
* Accurately document care provided and communicate promptly with the interdisciplinary team
* Participate in team meetings, in-services, and quality improvement activities as requested
Qualifications
Required
* Current Wisconsin Certified Nursing Assistant (CNA) certification in good standing
* Completion of a home health aide training program or ability to meet hospice/home health aide requirements
* Valid driver's license, reliable transportation, and adequate auto insurance coverage
* Ability to provide safe, respectful personal care in the home setting
* Dependable, compassionate, and comfortable working with patients and families during end-of-life care
* Basic documentation skills with ability to learn electronic medical record systems
* Previous hospice, home health, long-term care, or personal care experience preferred
* Comfort working independently in patients' homes while staying connected to the hospice team
* Strong communication skills and a caring, patient-centered approach
