Case Manager
Must be able to communicate and facilitate care along a continuum through effective resource coordination. Successful candidate will demonstrate strong communication and problem solving skills with a high degree of independent accountability and flexibility. HCBW case management experience preferred. Required degree: Social Worker, RN, or Bachelors degree in health care related field. Competitive wages and excellent benefits package including medical, dental, vision, 401(k) and more. Frequent travel covering surrounding area is required.
