Employment Type: 13-Week Contract with possibility of extension
Shift: Monday–Friday, 8:00 AM – 5:00 PM
Hours per Week: 40 Hours
Job Description
The Home Health RN works within the PACE (Program of All-Inclusive Care for the Elderly) model, a federal and state-funded program designed to provide comprehensive, coordinated, and proactive care to seniors, enabling them to live safely in their homes.
The RN is responsible for promoting and maintaining participants' health in the community through education, counseling, and coordination of preventive and rehabilitative services alongside an interdisciplinary team.
Key responsibilities include:
Assessing patient health needs
Developing, implementing, and overseeing nursing care plans
Providing direct nursing care in a home setting
Performing ongoing re-evaluation of individual and family needs
Coordinating scheduling, communication, and documentation
Ensuring efficient and effective home care service delivery
Requirements
Experience:
1–2 years of recent hospital nursing or acute care experience required
Home health and/or long-term care (LTC) experience preferred
Licenses & Certifications:
Active Registered Nurse (RN) license (must be currently active)
Valid driver’s license
Reliable transportation with proof of automobile insurance
Current CPR certification (BLS/BCLS required)
Skills & Abilities:
Strong prioritization and time management skills
Ability to manage competing demands and follow through consistently
Strong interpersonal, verbal, and written communication skills
Ability to communicate effectively in high-stress or emotional situations
Proficiency with EHR systems and Microsoft Office applications
Salary : $54 - $57
