PHHC is hiring a full-time case manager RN to manage cases and make home visits to our clients.. This is a great M-F job for those who enjoy providing one-on-one patient care and value client interaction. Perfect for self-starters with good assessment and planning skills and those who desire a flexible schedule and enjoy working independently. Join a strong, supportive team of nurses and therapists providing outstanding care in home health.

Compassion, time management, assessment, and computer skills required. Current Colorado or compact state nurse license, Valid Driver's license, proof of Motor vehicle insurance required. Experience in homecare preferred but new RN graduates welcome, as well as those new to homecare. We offer a full training program if you are new to the exciting field of home care.
