What You’ll Do
- Travel to different participant homes to deliver respectful person-centered support
- Build meaningful relationships with individuals, families, and team members
- Promote independence, dignity, and inclusion in every interaction
- Assist with daily living activities as needed (meals, hygiene, mobility, etc.)
- Support community participation, recreation, and skill-building
- Follow individualized care plans and document progress
- Maintain a safe, positive, and inclusive environment Document services and maintain compliance with all program standards
