Job Summary
Hope Hospice is seeking a compassionate and experienced Hospice Social Worker to join our healthcare team. The ideal candidate will provide emotional support, case management, and counseling services to patients and their families navigating end-of-life care. This role involves collaborating with interdisciplinary teams to ensure comprehensive patient-centered care, emphasizing dignity, comfort, and psychosocial well-being in a hospice setting. The Hospice Social Worker will utilize clinical expertise to assess patient needs, develop care plans, and facilitate access to community resources, all while maintaining strict adherence to HIPAA regulations and medical documentation standards.
Duties
- Conduct comprehensive psychosocial assessments of patients and their families to identify emotional, social, and practical needs related to hospice care.
- Provide individual, family, and group counseling sessions utilizing therapeutic modalities such as behavioral therapy, motivational interviewing, and crisis intervention.
- Assist with advanced care planning, including facilitating discussions around goals of care and end-of-life preferences.
- Coordinate with healthcare providers to develop and implement personalized care plans that address mental health concerns, addiction issues, dementia or Alzheimer’s care, PTSD, autism spectrum disorders, or other disabilities.
- Manage case documentation accurately within Electronic Medical Record (EMR) systems while ensuring compliance with HIPAA and other confidentiality standards.
- Support patients through crisis management and provide resources for community-based services such as assisted living or inpatient facilities when appropriate.
- Participate in clinical research activities related to hospice care or behavioral health outcomes to improve service quality.
- Facilitate intake processes for new patients and assist with medical records review using ICD-9/ICD-10 coding as needed.
- Collaborate with multidisciplinary teams including nurses, physicians, therapists, and social service providers to deliver holistic patient care across inpatient, outpatient, home & community settings.
Qualifications
- Bachelor's degree in Social Work (BSW) required; Master’s degree in Social Work (MSW) or related field preferred.
- Valid licensure or certification as a Licensed Clinical Social Worker (LCSW) or equivalent is highly desirable.
- Proven experience in hospice care, behavioral health, mental health counseling, or related healthcare settings such as hospitals or nursing homes.
- Familiarity with medical terminology, medical documentation standards, CPT coding, ICD coding (ICD-9/ICD-10), and EMR systems.
- Knowledge of specialized areas including dementia/dementia care, Alzheimer’s care, geriatrics, pediatrics, special education needs, working with developmental disabilities or autism spectrum disorders is advantageous.
- Strong skills in crisis management, case management, diagnostic evaluation, and working with diverse populations including children and seniors.
- Excellent communication skills with the ability to provide compassionate counseling tailored to individual patient needs.
- Ability to work independently within a multidisciplinary team environment while maintaining confidentiality per HIPAA regulations.
