Responsibilities
- Conduct comprehensive patient assessments and monitor vital signs to evaluate comfort levels and symptom management needs.
- Develop and implement personalized care plans aligned with patient wishes, medical conditions, and hospice guidelines.
- Collaborate with interdisciplinary teams—including physicians, social workers, and chaplains—to coordinate holistic end-of-life care.
- Provide direct nursing care in various settings such as inpatient units, outpatient clinics, or home visits, ensuring adherence to HIPAA regulations and medical records documentation standards.
- Facilitate discharge planning and educate patients and families on symptom management, medication administration, and advance directives.
- Utilize critical care experience, or emergency medicine expertise to manage complex cases involving acute or infectious diseases.
- Communicate effectively in both English and the designated second language (Spanish) to bridge language barriers and foster trust with diverse patient populations.
- Participate in utilization management and case management activities to optimize patient outcomes and resource utilization.
- Maintain accurate medical records, perform diagnostic evaluations, and support intake processes for new patients requiring hospice services.
Experience
- 1-2 years experience as Hospice RN.
- Preferably with background in post-acute care environments including inpatient hospice units or outpatient clinics.
- Knowledge of physiology, medical terminology, discharge planning procedures, and diagnostic evaluation techniques.
- Familiarity with managed care protocols, utilization management practices, and case management strategies.
- Experience working with diverse patient populations across , geriatrics, or infectious disease care is preferred.
- Strong understanding of HIPAA compliance standards along with excellent patient assessment skills in acute or outpatient settings.
- Bilingual proficiency(Spanish speaking) is essential to effectively communicate with patients from varied linguistic backgrounds while providing compassionate end-of-life support.
