What are the responsibilities and job description for the Case Manager position at Hope Haven?
Every child deserves the opportunity to realize their full potential. Through specialized services and individualized education, we help children and families navigate challenges, build skills, and access opportunities that support their success.
We are seeking a compassionate, curious, and resourceful Case Manager who can help families turn recommendations into meaningful action.
Families come to Hope Haven with diverse needs, circumstances, and challenges. Some are navigating a new diagnosis, others are seeking educational support, and many are trying to understand and access services across complicated systems. Our Case Manager helps connect the dots by working alongside families, our professional team, and community partners to identify needs, explore options, overcome barriers, and coordinate support.
What You'll Do
- Partner with children and families to understand their individual needs, priorities, and goals.
- Help families navigate educational, clinical, and community resources, including services available through Hope Haven and other organizations.
- Support families following evaluations or diagnoses by helping them understand recommendations, identify next steps, and access appropriate services.
- Provide educational advocacy and guidance related to IEPs, 504 plans, school accommodations, and educational transitions.
- Collaborate with psychologists, educators, therapists, behavior analysts, and other professionals to coordinate individualized support.
- Develop relationships with community organizations, service providers, and other resources that can benefit Hope Haven families.
- Follow through on identified needs and commitments, helping families overcome obstacles and make meaningful progress.
- Recognize recurring challenges, identify opportunities for improvement, and contribute to strengthening how Hope Haven serves children and families.
Who We're Looking For
We're looking for someone who combines empathy and strong interpersonal skills with initiative, sound judgment, and a genuine interest in solving problems.
The right person doesn't need to have every answer, rather they should be eager to find answers, ask thoughtful questions, seek out expertise, and follow through. This is a collaborative role that requires the ability to work independently, build relationships, manage competing priorities, and take ownership of responsibilities.
Hope Haven requires all potential new hires to pass a drug test and a Level 2 background screening through the Clearinghouse. For information about this screening you may go to https://info.flclearinghouse.com
