Schedule: Monday to Friday | Flexible hours | Some travel required

Job Summary:

We are seeking compassionate and motivated individuals to join our team as Case Managers and Community-Based Rehabilitation Specialists (CBRS). In this dual-role position, you will provide skill-building services in the community and support clients in accessing essential resources to promote mental wellness and self-sufficiency.

Key Responsibilities:

Community-Based Rehabilitation (CBRS):

• Work one-on-one with clients in the community to promote independent living skills.
• Assist clients with setting and working toward recovery-based goals.
• Provide support with tasks such as budgeting, social skills, scheduling, and coping strategies.
• Maintain detailed, timely documentation for all services provided.

Case Management:

• Link clients to community resources including housing, employment, transportation, and medical/psychiatric care.
• Advocate on behalf of clients to ensure access to appropriate services.
• Collaborate with treatment teams to coordinate client care plans.
• Monitor and evaluate client progress and adjust services as needed.

Requirements:

• Bachelor’s degree in Psychology, Social Work, or a related human services field (Required)
• Minimum 1 year of experience working in a behavioral health or mental health setting
• Strong written and verbal communication skills
• Valid driver's license, reliable transportation, and proof of insurance
• Ability to pass a background check
• Comfortable working independently in the community and in client homes

Preferred Qualifications:

• CBRS certification (or eligible for certification in Idaho)
• Knowledge of Medicaid documentation standards
• Familiarity with electronic health records (EHR) systems
