Position Summary
The MDS Coordinator is responsible for completing resident assessments, developing care plans, and ensuring accurate reimbursement in compliance with Medicare and Medicaid guidelines.
Key Responsibilities
- Complete and manage resident MDS assessments, including CAAs and triggers
- Coordinate interdisciplinary care plan meetings and ensure timely updates
- Develop and revise individualized care plans based on resident needs
- Support Medicare/Medicaid reimbursement processes and ensure accurate RUG coding
- Review clinical documentation to ensure compliance and proper billing
- Lead interdisciplinary care plan team meetings
- Conduct audits to ensure accuracy, timeliness, and compliance of MDS documentation
- Support survey readiness and respond to regulatory requests
- Train staff on RAI process and documentation requirements
- Maintain compliance with all federal, state, and facility policies
Qualifications
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) per state/facility requirements
- Completion of an accredited nursing program
- MDS, care planning, and assessment experience preferred (long-term care experience strongly preferred)
- Strong attention to detail and knowledge of Medicare/Medicaid guidelines
- Excellent communication, organization, and documentation skills
- Ability to work collaboratively with interdisciplinary teams

- Ability to lift up to 75 lbs and perform physical tasks as needed
- Must meet all state and federal regulatory requirements
- Strong interpersonal skills with energy, professionalism, and teamwork focus
