POSITION/ TITLE: Case Manager – Sullivan County

SUPERVISED BY: Director of Community-based Services

Shift Schedule: Monday - Friday, 10:00 AM - 6:00 PM

JOB DESCRIPTION:

Under the general supervision of the Director of Community-based Services, the Case Manager (CM) provides “Hope Through Hospitality” and will support the needs of individuals who are experiencing homelessness residing in various locations approved by the Sullivan County Department of Family Services (DFS), providing provisions of basic needs, and creating a safe, secure, and confidential residential living environment. The CM provides non-judgmental compassion and care by listening to client’s concerns and providing non-directive counseling. The CM acts as a role model, guiding clients and facilitating appropriate behaviors such as daily living skills, self-care, personal interactions, social relationships, constructive time management, following program policies and procedures. Must be flexible with schedule to meet the needs of the clients which will include some evenings and weekends. Additional hours may be required to meet program regulations, contract obligations and client needs.

QUALIFICATIONS FOR THE POSITION:

1. Bachelor’s Degree in Social work, Psychology, or related field.

2. Experience working in Human Service Field

3. Ability to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people including individuals recently incarcerated, recent crime victims, etc.

4. Integrity, positive attitude, mission-driven, and self-directed with demonstrated passion for HONOR’s mission.

5. Must have and maintain a valid New York State driver’s license.

6. Detail-oriented and have competent literacy and writing, documentation, communications, and interpersonal abilities (friendly, courteous, helpful, ability to work as part of a team).

7. Bilingual preferred but not required.

8. SOAR Certified within 90 days of employment.

Job Type: Full-time

Pay: $22.00 per hour
