- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to promote independence and dignity.
- Monitor patient health by checking vital signs, observing symptoms, and reporting changes promptly to healthcare professionals.
- Support medication administration following care plans and HIPAA regulations to ensure safety and compliance.
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary.
- Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing personal spaces.
- Use specialized equipment such as Hoyer lifts or transfer belts to safely assist with mobility and prevent injury during transfers.
- Document care activities accurately in compliance with facility policies and coordinate with healthcare teams to update care plans regularly.
- Proven experience in home health, assisted living, nursing homes, or group home settings is highly desirable.
- Certifications in First Aid, CPR, and HIPAA compliance are required; additional training in Alzheimer's care, dementia care, or hospice care is a plus.
- Ability to work effectively with individuals with developmental disabilities, autism spectrum disorder, or memory impairments such as dementia.
- Strong observational skills for patient monitoring and recognizing changes in condition or behavior.
- Physical ability to perform heavy lifting tasks safely and assist with patient transfers using appropriate equipment like Hoyer lifts.
- Excellent communication skills for working collaboratively with clients, families, and healthcare professionals.
