Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and health history reviews to develop individualized care plans.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and medication administration for chronic conditions.
- Monitor residents’ health status continuously, recognizing signs of acute pain, infection, or deterioration requiring immediate intervention.
- Coordinate with interdisciplinary teams to facilitate treatments such as wound care, suturing, ostomy care, or dialysis as needed.
- Maintain detailed medical documentation in electronic health records (EHR/EHR systems), ensuring compliance with HIPAA regulations and accurate coding using ICD-10/ICD-9 standards.
- Provide emotional support and health coaching to residents and their families, fostering a caring environment that promotes independence and dignity.
- Participate in discharge planning, case management, and collaboration with physicians for complex cases involving trauma medicine or behavioral health needs.
Requirements
- Valid RN license in good standing with current state registration.
- Proven experience working in assisted living facilities or long-term care environments; prior nursing home or memory care experience is highly desirable.
- Strong knowledge of medical terminology, anatomy, physiology, and aseptic techniques essential for safe patient care.
- Proficiency with EMR/EHR systems such as Cerner or Athenahealth; familiarity with medical documentation standards including CPT coding is preferred.
- Experience managing medication administration including injections, IV therapy, tube feeding, and ventilator management if applicable.
- Critical care experience such as ICU or emergency room background is advantageous; familiarity with trauma center protocols is a plus.
- Ability to perform physical examinations, laboratory specimen collection/processing, phlebotomy, and wound suturing when necessary.
- Excellent communication skills for triage assessments and collaboration within multidisciplinary teams.
- Knowledge of infection control practices and compliance with NCQA standards for quality healthcare delivery.
