Job Summary

Are you a master of logistics with a heart for helping others? We are seeking a dedicated and organized Field Service & Intake Coordinator to join our growing home health team in Lewistown, PA.

In this vital role, you will serve as the central link between our field clinical staff, patients, families, and community referral sources. You will manage the vital first step of a patient's journey by processing incoming referrals, while simultaneously coordinating day-to-day field logistics. Key Responsibilities

• Intake & Referral Processing: Serve as the primary point of contact for hospitals, physicians, and community partners. Efficiently gather, review, and process incoming patient referrals for home health services, verifying insurance eligibility and clinical documentation.

• Scheduling & Coordination: Manage, build, and optimize daily schedules for field staff to ensure all patient visits are covered efficiently and remain strictly compliant with established care plans and initial start-of-care timelines.

• Field Support & Logistics: Act as the primary point of contact for field clinicians, troubleshooting schedule changes, patient status updates, or geographical and logistical hurdles in real-time.

• Patient Onboarding & Advocacy: Reach out to newly referred patients and families to introduce services, confirm scheduling availability, address concerns, and maintain ongoing empathetic contact to ensure care satisfaction.

• Documentation & Compliance: Accurately input and maintain all intake data, authorization records, scheduling details, and patient files within our Electronic Medical Record (EMR) system, ensuring full adherence to agency policies and Pennsylvania state regulations.

Qualifications

• Experience: Minimum of 2 years of experience in Home Health scheduling, intake, coordination, or a closely related healthcare administrative role (Required).

• Education: High School Diploma or equivalent required (Associate’s degree in healthcare administration or a related field preferred).

• Geographical Knowledge: Strong familiarity with the geography and roadways of Lewistown and the surrounding Mifflin County area to effectively route field staff.

• Technical Skills: Proficiency with Electronic Medical Record (EMR) systems, insurance verification portals, and the Microsoft Office Suite.
