What are the responsibilities and job description for the Targeted Case Manager position at HoneyBee Community Services?
This is a hybrid position
Person-Centered Planning:Work with individuals to develop a Person-Centered Support Plan (PCSP) that aligns with their goals, preferences, and what is important to them.
- Service Access:Assist individuals in accessing and receiving a range of needed services and supports, including day programs, residential care, and employment support, as outlined in the IDD Waiver.
- Advocacy:Act as an advocate for individuals to ensure they receive the appropriate services and supports that best meet their needs and help them achieve their goals.
- Service Monitoring:Monitor and evaluate the services in place to ensure they are meeting the client's needs and effectively supporting their preferred lifestyle.
- Community Resource Connection:Connect individuals with community resources beyond the waiver to promote independent living and community involvement.
- Coordination:Coordinate various aspects of care with different service providers to improve continuity of care and overall outcomes for the individual.
Job Type: Full-time
Pay: $20.00 - $30.00 per hour
