The Care Coordinator is responsible for coordinating, implementing, and supervising the delivery of services to agency clients in alignment with the company’s mission and standards of care. This position is accountable for adherence to regulatory and agency requirements for service provision and ensures compliance with and maintenance of client records.
Responsibilities
- Collaborate with physicians, patients, families, and healthcare staff
- Coordinate a variety of healthcare programs
- Develop individualized care plans
- Educate patients on their healthcare options
- Create goals and monitor progress toward goals
- Create schedules
- Minimum qualification: Certified Nursing Assistant (CNA) required
- Minimum 2 years of home care field experience required
- Experience in care coordination, supervision, case management, or client services preferred
- Experience conducting client assessments and supervisory visits
- Experience supporting caregivers and monitoring service quality
- Experience with scheduling, documentation, and compliance
- Strong communication, customer service, and relationship-building skills
- Ability to represent the agency professionally and promote services to referral sources
- Strong organizational and time management skills
- Ability to work independently and as part of a team
- Valid driver’s license
- Willingness to travel within the service area
- Basic computer skills and the ability to complete documentation accurately
