We’re looking for a compassionate, organized, and proactive Care Manager who wants to be part of building an exceptional home care organization from the ground up.

What You’ll Do

As a Care Manager, you’ll serve as the primary point of contact for our clients and their families while ensuring every client receives outstanding care.

Responsibilities include:

• Conduct in-home client assessments
• Develop individualized care plans
• Match caregivers with clients based on skills, personality, and client needs
• Build trusted relationships with clients and family members
• Perform ongoing quality assurance visits
• Monitor client satisfaction and proactively resolve concerns
• Coordinate changes in care plans as client needs evolve
• Maintain accurate documentation and client records
• Communicate regularly with caregivers, families, and healthcare professionals
• Build relationships with hospitals, rehabilitation centers, assisted living communities, physicians, and other referral partners throughout Stamford and Greenwich

What We’re Looking For

Preferred qualifications:

• Experience in home care, healthcare, senior services, case management, or care coordination
• CNA, HHA, LPN, RN, Social Work, or related healthcare experience welcomed
• Excellent communication and interpersonal skills
• Strong organizational and time-management abilities
• Ability to prioritize multiple responsibilities while remaining calm under pressure
• Professional, compassionate, and dependable
• Self-starter who takes initiative without waiting to be directed
• Comfortable working independently while collaborating with a growing team
• Valid driver’s license and reliable transportation
