We are looking for a caring and dependable Home Health Aide to deliver high-quality, in-home support to individuals with a range of needs. The right candidate is committed to providing patient-focused care and has experience helping with daily living activities (ADLs), maintaining accurate medical records, and working in home health or long-term care settings. In this important role, you will offer individualized assistance that enhances clients’ independence, ensures their safety, and supports their overall health and quality of life.
Duties
- Help clients with everyday activities such as bathing, dressing, personal hygiene, and mobility, including the safe use of equipment like Hoyer lifts when needed.
- Observe and track patients’ conditions, recording vital signs, medication details, and any health changes accurately within EHR or EMR systems.
- Deliver specialized support for individuals with dementia, Alzheimer’s, or other cognitive conditions using appropriate memory care approaches.
- Provide assistance in both assisted living facilities and private residences by preparing healthy meals, keeping living spaces clean, and handling laundry.
- Administer medications according to established guidelines while maintaining strict adherence to HIPAA standards for patient confidentiality.
- Support clinical tasks such as catheter care, hospice services, and wound care under the direction of licensed nursing staff.
- Interact with clients in a compassionate and respectful manner, promoting dignity, comfort, and independence.
- Work closely with healthcare teams to coordinate care and ensure accurate and thorough documentation.
Qualifications
- Must be a certified Home Health Aide (HHA) or Personal Care Aide (PCA).
- Must speak English fluently.
- Valid ID and Social Security Card.
- Two professional references.
- Physical exam and up-to-date immunizations (MMR).
- Tuberculosis test and drug screening.
- Must be able to commute to the patient’s home.
