This role serves as a key resource for patients, providers, and support staff by monitoring patient data, addressing alerts, and ensuring accurate documentation. The specialist plays a critical role in patient safety, engagement, and compliance and has an immediate opportunity to make a meaningful impact.Qualifications & AttributesEducation & ExperienceHigh school diploma or GED requiredPost-secondary education preferredProven experience working in a medical office or healthcare environmentWorking knowledge of medical terminology and medicationsTechnical SkillsProficiency in Microsoft Office (Word, Excel, Outlook)Strong computer and electronic medical record (EMR) skillsAccurate typing and filing abilitiesReliable broadband internet connection (Wi-Fi required for remote work)Core CompetenciesExcellent written and verbal communication skillsStrong interpersonal and presentation skillsOutstanding organizational and time-management skillsStrategic thinking and analytical abilitiesProblem-solving, critical thinking, and sound decision-making skillsAbility to work independently and manage multiple prioritiesMajor Areas of ResponsibilityPatient Monitoring & SupportProvide continuous observation and support for patients enrolled in the RPM programDemonstrate proficiency with Remote Patient Monitoring software systemsMonitor patient vitals and identify high-risk alerts or changes in patient conditionEnsure patients remain connected (Bluetooth) to monitoring devices and monitor dailyReview weekend and weekly data and ensure patient compliance with program requirementsCommunication & CoordinationCommunicate patient alerts and concerns to providers promptlyRespond to incoming calls and return voicemails in a timely and professional mannerAssist patients with device usage, troubleshooting, and general questionsServe as a resource for providers and internal support staffDocumentation & ComplianceDocument patient interactions and monitoring activities in the EMR per departmental policyAccurately record all activities in CCM/RPM time-tracking softwareIdentify and enroll eligible patients into the RPM program and ensure proper device distributionAdditional ResponsibilitiesMaintain compliance with departmental policies and proceduresComplete additional tasks as assigned by the RPM ManagerDecision-Making AuthorityThe RPM Support Specialist is expected to exercise sound judgment and independently identify and resolve routine issues while escalating concerns appropriately.RPM Inventory ManagementAssist with the management and organization of Remote Patient Monitoring (RPM) inventoryPrepare RPM kits by assembling Ensure devices are properly labeled, tested, and configuredCoordinate shipping of RPM kits Maintain accurate inventory recordsProcess returned RPM equipment
