Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
- Monitor and document patient observations, vital signs, and medical conditions accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Administer medications and perform specialized care such as catheter care, hospice support, Alzheimer’s and dementia care, and memory care routines
- Support clients with meal preparation, cooking, laundry, cleaning, and maintaining a safe home environment in compliance with health standards and HIPAA regulations
- Conduct thorough patient assessments, report changes promptly, and maintain detailed medical documentation to ensure continuity of care
- Work effectively with interdisciplinary teams to coordinate care plans tailored to individual needs and preferences
- Demonstrate compassion and patience when working with people with developmental disabilities or disabilities requiring long-term care
Requirements
- Valid certification as a Home Health Aide (HHA) or equivalent credential recognized by state licensing agencies
- Proven experience in home health care, nursing homes, assisted living facilities, or long-term care environments
- Knowledge of medical terminology and procedures related to dementia, Alzheimer’s disease, hospice, and memory care services
- Ability to operate EMR/EHR systems efficiently for documentation and reporting purposes
- Current CPR certification; additional certifications such as HIPAA training are highly desirable
- Experience working with people with disabilities or developmental challenges is preferred
- Strong communication skills to effectively collaborate with clients, families, and healthcare teams
- Flexibility to work live-in shifts or flexible hours as needed
