The Role
As an Aging Services Consultant you'll conduct Care Consultations, develop personalized care plans, lead Care Pro introductions, and conduct quality assurance visits — all while serving as a steadfast advocate for clients and their families throughout their care journey.
What You'll Do
- Respond to incoming inquiries and conduct compassionate, consultative Care Consultations.
- Develop individualized care plans and convert consultations into active care starts.
- Lead Care Pro introductions and conduct ongoing quality assurance visits.
- Maintain consistent communication with clients and families throughout the care relationship.
- Document all touchpoints accurately in WellSky and update care plans as needs evolve.
What We're Looking For
- Experience in client services, healthcare, senior care, or social work.
- Natural relationship-builder with strong listening and communication skills.
- Consultative approach — you know how to guide and connect, not just pitch.
- Organized and detail-oriented with the ability to manage multiple clients at once.
- Valid driver's license and willingness to travel to client homes.

Pay: $50,000.00 - $55,000.00 per year
